BAYADA Home Health Care, Inc. is proud to support your journey to becoming a Licensed Practical Nurse (LPN) or Registered Nurse (RN). As a participant attending ECPI University, your scholarship funding will be managed by The Beacon Education Foundation in partnership with ECPI University. While BAYADA is not disbursing tuition funds, we are committed to supporting your academic and professional success through ongoing coaching and verification of your employment.

Consent and Acknowledgment

I understand that I am applying to participate in the Advance to LPN program and that my scholarship is being managed by ECPI University. I agree to the following:

1. I will maintain employment with Bayada.
2. I will participate in monthly coaching sessions with a BAYADA coach.
3. I consent to BAYADA sharing my employment status and work hours with ECPI
   University.
4. I understand that my continued scholarship eligibility through ECPI is dependent on
   fulfilling these conditions.
5. I authorize ECPI University and The Beacon Education Foundation to share relevant
   information with BAYADA to support program participation, including enrollment and
   graduation verification, in accordance with FERPA guidelines.
